Select the size of the center hole:

• Medium/Standard: 1.875" W x 1" D
• Large: 2.625" W x 1" D

Most will use Medium, as you want the fit tight enough that the testicles cannot slip through, however they should also not be pinched or crushed. To measure, pull the testicles downward (at least 2" of stretch) to desired tension , and measure the width of the scrotum at the leg line.
